This is a typical symptom of a FITV that needs to be adjusted. There is a piece in the FITV that can come unscrewed and will cause the problem you are describing.

So the idle surging only occurs while moving? When you come to a stop the surging stops?

I replaced the vacuum lines running to the MAP sensor yesterday and have not experienced the problem again. I will put an update if the problem occurs again.

So the link you provided makes it sound like upgrading a v3 file should be no problem but it does not address v4 files?
